- Driver of the other vehicle (if applicable)
- Date of the accident
- Location of the accident
- What local police were contacted (if any)
Massachusetts requires that you complete a Motor Vehicle Crash Operator Report for any accident involving injury and/or physical damages exceeding $1,000. One copy must be sent to the Registry of Motor vehicles, one to the police department with jurisdiction over the location where the accident occurred, and one to your insurance company.
